Notarization Services in Hospitals and Assisted Living Facilities
Being a patient or family member assisting a loved one in a hospital or an assisted living facility is not easy and can be very stressful. My goal is to make the process as easy as possible, not only for the patient/resident but also for their loved ones. To do this, there are a few requirements when I offer my notarization services in Turlock, CA, and the surrounding areas.
Please be considerate of the hospital or assisted living facility's schedule when arranging the visit of a notary agent. Inquire about visitors prior to scheduling and the number that are allowed in the room. For assisted living, find out if there is a place that is convenient for the signing. If we make it easy for the facility, they will make it convenient for us.

Requirements:
- The signer must be alert and understand what they are signing.
- The document(s) requiring notarization must be present.
- The signer must be able to communicate with me. I'm not bilingual and do not offer such services.
- The signer should have a current government-issued driver's license, ID card, or passport. If one of these is not available, the other option is to have two credible witnesses in order to identify the signer.
- The two witnesses must be impartial to the transaction.
- The witnesses must present a current government-issued ID (driver's license, ID card, or passport).
- The witnesses must take an oath or affirmation to the identity of the signer.
- The witnesses must sign the notary journal affirming that they identified the signer.
- The signer must be able to sign the document or "signature by mark." One of the witnesses will be required to sign the document that he/she witnessed the signer's mark.
- All medications that would inhibit cognitive awareness must be withheld for an hour prior to the scheduled appointment.
The notarization will not take place if the signer shows any indication of being impaired or under pressure, duress, or coercion. It is common to ask all non-medical personnel to step out of the room to assess the situation.

The signing must be witnessed by an ombudsman known as a "patient advocate."
Prior to scheduling, contact the facility to find out when an ombudsman is available. The ombudsman has the authority to approve the signing taking place and to determine that the patient is/is not capable of making decisions or is physically unable to sign.
